How do I earn my Wellness Discount?

Submit Proof of Completion

Any care team member who completes the following activities between November 1, 2023 - October 31, 2024 will qualify for a 10% Wellness Discount, if they are enrolled in the Core Advantage EPO medical plan for 2025

• Annual Physical Exam completed by your Primary Care Physician
  

• Associated Labs as ordered by your Primary Care Physician
  

*** Wellness Points will also be earned for completion of these activities.

During your annual visit with your primary care physician, present the 2024 Physical Exam & Lab Work - Proof of Completion Form for a verification signature of services provided. Then submit completed form to the 4URHealth office:

***Alternative forms of documentation proof will be accepted if your physician requires a fee to complete form.

The deadline to complete these activities and submit your proof of completion is OCTOBER 31, 2024.
